Leroy Eugene Chism, III was born on November 14, 1979 to Annette Crockett and Edward Chism, Sr. in Kansas City, MO.

He was preceded in death by his mother, Annette Crockett; his brother, Edward Chism, Jr.; his sister, Eugene Chism; both maternal and paternal grandparents, Rochester and Hazel Spratt, Leroy, Sr. and Gladys Chism; uncles, Leroy Chism, Jr., Jackie Chism and Edmond Crockett; and aunt, Nancy Chism.

At an early age, Leroy united with the Prince of Peace Missionary Baptist Church.  Then, later in life, he completed his education at the Kansas City Public Schools.

He leaves to cherish his memories his son, Matthew Chism of the city; his father, Edward Chism, Sr. of Morrow, GA; stepfather, Ernest Crockett of the home; sisters, Erica and Tiffany Crockett of the city; brother Marklo Baker of the city; aunts, Theotrice, Gladys, Shellie, Mary, and Loretta; uncles, Charles and Rodney Crockett; cousins, nieces, nephews, great aunts and uncles.
